The purpose of this study was to examine relationships among neighborhood socioeconomic status, neighborhood social capital, and female adolescent reproductive health promotion and to test a model of neighborhood social capital as a mediator of neighborhood socioeconomic status on female adolescent reproductive health promotion. A cross-sectional, correlational design was used for this study. A secondary data set analysis of Wave I of the National Longitudinal Survey of Adolescent Health was conducted utilizing structural equation modeling to test the model. The sample included 2612 sexually active girls with completed parent questionnaires and neighborhood contextual data.;Final model testing yielded acceptable fit of the model to the data with a chi2 = 285, df = 67, p < .001, CFI = .95, and RMSEA = .04. Statistically significant direct paths were found from neighborhood socioeconomic status to trust (beta = .09, standardized), network membership (beta = .09 standardized), and norms of reciprocity (beta = .09 standardized); from trust and network membership to attitudes towards pregnancy (beta = .06 and .10 respectively, standardized); from attitudes towards pregnancy to attitudes towards contraception and condom use (beta = .20 and .17 respectively, standardized); and from attitudes towards contraception to condom use and highly effective contraceptive use (beta = .29 and .31 respectively, standardized). Trust and network membership were statistically significant mediators of neighborhood socioeconomic status on attitudes towards pregnancy. Data on this area are particularly scarce in China, where millions of FSWs play an important role in the escalating HIV epidemic. This book has three aims: first, to investigate the present knowledge on behavioral studies of FSWs in China and identify the literature gaps; second, to examine the risk factors associated with FSWs' suicidal ideation and attempts; and third, to explore the relationship between depressive symptoms and FSWs' condom use behaviors. The first study is based on a systematic review of English literature regarding FSWs in China. The second and third study draw data from a community-based HIV/STD intervention study with FSWs in Guangxi, China. The data reveal that FSWs in China have poor psychological wellbeing with high rates of suicidal behaviors and depression. Their mental health problems are associated with HIV- related sexual risks. Health education and promotion efforts among FSWs in China need to take into consideration their level of psychological distress.

Prevalence of sexually transmitted infections and HIV and associated risk factors among female sex workers in Guangdong Province, China.

Female sex workers (FSWs) play an important potential bridging role in the epidemic of HIV from specific high-risk populations to the general population. A study was conducted to understand their risk behaviors, their HIV and STD infection status, and the related predictors for STD infection among FSWs in Guangdong Province, China. Qualitative quantitative studies were combined to elucidate risk behaviors among FSWs. In the quantitative study, respondent-driven sampling (RDS) was used to recruit 320 FSWs in a city in Guangdong Province, China. The recruited participants were interviewed face-to-face, using a structured questionnaire, then tested for HIV, syphilis, gonorrhea, and Chlamydia. The median number of clients in the past week among FSWs was five. The rate of consistent condom use with the clients during the last week was 58.0% (50.4, 65.5%), but and much lower regular non-paying and non-regular non-paying partners. Street-based FSWs had more risk behaviors for HIV and STD infection. The proportion of inconsistent condom use with clients and with regular non-paying partners was lower than that for establishment-based sex workers. None of the participants was found to be HIV-positive. The prevalence of syphilis, gonorrhea, and Chlamydia was 8.0% (4.6-12.2%), 9.5% (5.6-14.3%) and 3.9% (1.7-6.2%), respectively. The prevalence of having at least one of these infections was 19.7% (13.9-26.2%). The multivariate logistic regression model showed that years of education, awareness of HIV/AIDS, risk of oneself for HIV infection, amount last charged for commercial sex, regular sex partners making the decision about condom use, and the venues where the participants met clients were independently associated with STD infection. The future intervention program should target street-based FSWs.

This dissertation research examined factors associated with the decision to obtain an HIV test among Chinese/Chinese American community college women and explored the association among relationship status, partnership communication, and discussions of HIV testing.;Using constructs of the health belief model, an online and paper-pencil, cross-sectional design survey was administered at four community college campuses in Northern California (N=230). Results indicate 30% of respondents obtained HIV testing. The most common reasons for testing were "just to find out" (73%), "having had unprotected sexual intercourse" (63%), and "having had sex with a new partner" (57%). Among those who never tested, low levels of perceived susceptibility and lack of knowledge on testing sites were the most common barriers. Multiple logistic regression analyses revealed older age, American-centered/mixed ethnic identity, lack of condom use during last intercourse, lower perceived barriers, and higher self-efficacy as significant predictors of HIV testing history (p = .007). Emphasizing these reasons in HIV education campaigns will likely increase testing rates for this population.;In regards to partnership status, 60% of respondents were in committed partnerships, 21% in casual partnerships, and 19% were single or in nonsexual relationships. Twenty-five young men between the ages of 13 and 27 were interviewed for this study. They share with you their views on: sexual initiationsexual definitionsexual orientationlovedrug useprostitutionfamily relationshipsrelationships with men and women The young men interviewed for this study are in serious danger of being exposed to the AIDS virus and of becoming addicted, if they are not already, to cocaine, crack, or alcohol. Those conducting the study initiated a campaign to supply condoms and raise the young men's awareness about AIDS and drugs and began an immediate support program. The project resulted in the establishment, in June 1997, of an alternative home for juvenile prostitutes, which offers various opportunities for education and work.

This study focused on psychosexual and interpersonal constructs and their relationship with partner risk and unprotected intercourse, based on the multifaceted model of HIV risk (MMOHR). The model was examined for 625 men and women (ages 18-24); the model was then compared across gender. The MMOHR-based model was strongly supported. Having a higher sense of self-efficacy in using condoms, and a higher sense of assertiveness in practicing prevention both predicted decreased unprotected vaginal intercourse. Contrastingly, those better able to refuse unwanted sexual advances were more likely to report unprotected vaginal intercourse. Condom use assertiveness significantly predicted condom self-efficacy and condom negotiation. Having higher satisfaction and sense of control over one's sex life reduced partner risk behaviors as well as condom negotiation. These findings suggest that although having the skills to communicate condom use is important, being invested in prevention is a stronger influence on whether the person engages in protected vaginal intercourse. The negative relationship between refusal assertiveness and unprotected vaginal intercourse suggests that if people are unable to avoid unwanted sexual contact, perhaps they are more diligent in using condoms, but if they are able to avoid unwanted sexual attention, then they are less likely to practice protected intercourse. Most of the parameters predicting the risky sex behaviors were found to be comparable across gender. However, the relationship between condom use assertiveness and condom negotiation was significantly different. Additionally, men who were more satisfied with their sex life were less likely to endorse condom self-efficacy or condom negotiation, while women who felt more assertive in refusing unwanted sexual attention were more likely to report satisfaction over their sex life. These findings suggest that although men and women may be similar in whether they engage in risky sex, there
